AHLOA membership dues for calendar year 2019 are now payable.  If you have not already renewed using the package mailed to your address of record, please print and complete the Enrollment Form below and remit a check payment for $20 payable to “AHLOA”.  Please mail your enrollment form to the following address:

 If you have already paid your dues for 2019 and have not supplied detailed information, we would appreciate it if you printed, completed and mailed the attached so we can update our database, eliminate old and inaccurate information, and improve our future communication with you.
